ICB – Warm Homes Project

The Warm Homes project is a joint initiative between the Norfolk and Waveney ICB and local councils.

The Warm Homes project will identify and contact clinically and socially vulnerable patients offering advice and onward referral to Great Yarmouth Borough Council and East Suffolk Council for financial support to reduce exacerbations of chronic respiratory ill health caused by living in cold homes and fuel poverty.

The patients we will be contacting are children aged 0-14 years with asthma (engaging with parent/carer within household), 60-85 year olds with COPD or asthma and patients of all ages with cystic fibrosis. All patients will be living in households in our most deprived areas of Great Yarmouth and Waveney.

Letters will be sent out offering support and information, followed by calls by our team of motivational coaches within the virtual support team to engage with patients and identify if they are struggling with paying for essentials, e.g. energy or food and would like support from their local council.

Protect NoW with the support of Great Yarmouth Borough Council and East Suffolk Council will commence contacting patients from the end of November onwards. If you have any questions or you wish to opt out, please contact nwicb.phmi@nhs.net by 21st  November 2023.
